We ask parents and guardians to submit the education documents of their children with albinism. We want IEPs, IFSPs, and 504s for children from preschool to high school. Because state regulations may differ, we will also be categorizing our documents by state. If you have an original electronic copy in a word processing form, that is the best submission option. However, a scanned copy will also work. To submit the documents you can either email your documents to the IEP Albinism Team or send them to the NOAH office at PO Box 959, IEP Project, East Hampstead, NH 03826-0959. Include this form so that we may contact you and classify your submissions by the appropriate grade and state. The team will scrub your documents for personal information, extract goals and assistive technology for our databases, return them to you for a final review and parent comments, and publish your scrubbed documents on this site. |
